Your upper back teeth sit directly beneath the maxillary sinus. When those teeth are lost, the sinus floor tends to drop down into the empty space while the jawbone shrinks from below. Dentists call this sinus pneumatization. It is usually painless, but it steadily removes the bone a dental implant needs, which is why it is one of the less obvious consequences of tooth loss and one of the most important to catch early.
How Your Teeth And Sinuses Are Connected
The paranasal sinuses are air-filled spaces in the bones of the face. There are four paired groups: the frontal sinus above the eyes, the ethmoid and sphenoid sinuses behind the nose, and the maxillary sinus in each cheek. The maxillary sinus is the largest, and its floor sits right above the roots of the upper premolar and molar teeth.
In many people the bone separating those tooth roots from the sinus is very thin. On a CT scan, some roots appear to project into the sinus, covered only by the sinus lining (the Schneiderian membrane). This close relationship between teeth and sinuses explains a few things patients often notice:
- A sinus infection or allergy flare can make healthy upper teeth ache, because the inflamed sinus lining presses on nerves that also serve the teeth.
- An infected upper tooth can drain into the sinus and cause sinus symptoms of its own, sometimes called odontogenic sinusitis.
- Removing an upper back tooth changes the anatomy of the sinus floor, and that change continues for years after the extraction.
If you have pain in your upper teeth along with congestion or pressure in the cheek, it is worth having both a dental exam and, where needed, an evaluation by an ear, nose and throat specialist so the true cause is found.
What Is Sinus Pneumatization?
Pneumatization simply means the growth of an air space within bone. It is a normal developmental process: the maxillary sinus enlarges through childhood and continues to increase in size and volume until the permanent teeth erupt. The final size varies from person to person. Normal pneumatization can be larger on one side than the other, and the opposite pattern, sinus hypoplasia (an underdeveloped sinus), is also a recognized anatomical variant.
What concerns us as periodontists is the pneumatization that happens after tooth loss. Once the upper posterior teeth are gone, the floor of the maxillary sinus often moves down into the edentulous (toothless) area. One commonly proposed explanation is that the teeth and their chewing forces help maintain the surrounding bone, and without them the sinus is free to expand into space. At the same time, the ridge that held the teeth resorbs from below. The bone is being lost from two directions at once.

Can Missing Teeth Cause Sinus Problems?
Sinus expansion on its own does not usually cause symptoms. Most patients never feel it happening, and it does not typically lead to chronic sinusitis by itself. The real problem is structural: by the time someone comes in for an implant consultation, there may be only a few millimeters of bone left under the sinus in the area where the tooth used to be.
The reverse question matters too. Many patients with a history of sinus trouble ask whether it rules out implants. In Dr. Nejat’s experience, sinusitis, allergies and a deviated septum are not contraindications to dental implants. They are simply part of the medical history we review when planning treatment.
Why Sinus Pneumatization Matters For Dental Implants
A dental implant needs enough bone height and width to hold it securely. When maxillary sinus pneumatization has taken much of that height, an implant cannot simply be placed in the upper back jaw without a plan for the missing bone. This is where three-dimensional imaging becomes essential. A cone beam CT scan lets us measure the remaining bone, see the floor of the maxillary sinus and the sinus lining, and check for any sinus disease before surgery.
Depending on what the scan shows, the main options are:

Many patients arrive having been told elsewhere that zygomatic implants are their only option. More often than not, we find an alternative, such as a sinus graft combined with a pterygoid implant behind the sinus. The right choice depends on your anatomy, and our periodontists have placed more than 10,000 implants across these approaches.
How To Limit Sinus Expansion After An Extraction
Timing makes a real difference. The longer an upper back tooth has been missing, the more bone is typically lost. If you need an upper molar or premolar removed, ask about socket and bone preservation at the time of extraction, and plan the replacement early rather than leaving the gap open for years.
